Description

JOB SUMMARY: The Dietitian Assistant is responsible for overseeing various nutrition-related tasks, including managing supplement orders and deliveries; coordinating frozen meal services; triaging referrals; and facilitating educational group activities.

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

  1. Supplement Management – serve as the primary contact for refill requests; track supplement deliveries; monitor utilization; and coordinate pickup and delivery.
  2. Frozen Meal Management – placing orders; track meal deliveries; monitor utilization; and coordinate pickup and delivery.
  3. Referral and Scheduling Coordination – triage dietitian referrals based on urgency and manage Registered Dietitian schedule.
  4. Serve as the primary point of contact for issues and concerns that do not require a Registered Dietitian.
  5. Coordination with other departments, such as home care, regarding special diet orders.
  6. Ensure appropriate staff are trained on modified diet consistencies.
  7. Provide educational group activities, such as cooking groups.
  8. Coordination of Nutrition Meetings – serve as the primary point of contact for setting the agenda; coordinating meeting times; and acting as a liaison between HSRC Nutrition and PACE.
  9. Travel between PACE sites.
  10. Acts in a manner consistent with agency values.

Qualifications


EDUCATION &/or EXPERIENCE:

  1. High school diploma or GED equivalent required.
  2. Two (2) to three (3) years of administrative experience required, preferably in the health care field.
  3. Minimum of one (1) year experience working with a frail or elderly population preferred.
  4. Experience with facilitating groups and/or teaching, preferred.

 

LICENSES OR CERTIFICATES:

  1. Valid California Driver’s License and personal auto liability insurance preferred. Personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above preferred.
  2. Current CPR and First Aid certification within six (6) months of hire.

 

PRE-SCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Pre-employment physical and PPD skin test.
  3. Requires Motor Vehicle Report verification to determine eligibility of driving record.
  4. COVID-19 and Influenza (flu) vaccines preferred.

Demonstrates effective social interaction with co-workers, management, participants, and community contacts including referral sources.
  3. Ability to work independently, with minimal supervision, and within an Interdisciplinary Team.
  4. Strong organizational and time management skills.
  5. Ability to complete duties within an agreed upon time frame and to adjust personal schedule if required, adapting appropriately to changes in priorities and/or workload and incorporating an understanding of detailed requirements into action plans and implementation.
  6. Demonstrates sound verbal and written communication skills to convey information effectively.
  7. Maintains a positive attitude and balance in relationships with others in complex interpersonal situations.
  8. Ability to keep immediate and surrounding work area neat and organized, in a manner consistent with a professional front-office reception environment.
  9. Ability to maintain excellent attendance and punctuality records.
  10. Ability to work in a constantly changing environment with individuals of various backgrounds and abilities.
  11. Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and relevant computer programs and software (e.g. electronic health record software). Basic knowledge of web-based applications such as timekeeping, training, and email programs.

 

PHYSICAL REQUIREMENTS:

  1. Physical abilities sufficient to move between different staff work areas, communicate with co-workers and the public, operate a computer, produce reports, talk on the telephone, and travel to other agency worksites.
  2. Ability to sit at a desk and interact with a computer screen for extended periods of time.
  3. Hand and arm strength sufficient to operate a keyboard for several hours each day.
