JOB SUMMARY: Performs duties that fall into five general categories: participant care, documentation, coordination, team support, as well as advocacy and education. This position can be part or full time. The schedule is Monday through Friday between the hours of 8 am and 5 pm.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Administers treatments in accordance with physician orders and/or as directed by the individual plan of care (IPC).
- Monitors, administers, and records prescribed medications per Title 22.
- Responsible for managing medical emergencies.
- Provides other nursing services as directed by program management.
- May be requested to conduct home visits as needed to assess participant status.
- Maintains skilled nursing care records, completes daily nursing flowsheets, progress notes, medication administration records, and other medical data as necessary to document medical and nursing care.
- Completes agency incident reports and participant fall reports and implements measures to prevent reoccurrence. Submits reports to program management and notifies participants’ authorized representatives and physician of all participant incidents.
- Monitors safety of the Day Center work environment and safety practices.
- Assists Social Workers and Program Director (Social Services Program Manager) in facilitating team communication regarding participant, caregiver, employee, and program issues.
- Generates appropriate referrals and coordinates with Registered Dietitian, Physical Therapist, Occupational Therapist, Licensed Clinical Social Worker, and Speech Therapist to meet participant needs and care plan goals.
- Participates in the process of teaching, training, and mentoring CarePartner staff, student interns, and volunteers working within the program area.
- Assures proper medication storage and destruction, monitors medical supply inventory and functioning equipment, and follows medication administration procedures in compliance with Title 22 regulations and all applicable laws and regulations.
- Performs routine quality control checks on medical equipment and documents in compliance with Title 22 regulations and all applicable laws and regulations. Follows up to repair and replace equipment as needed.
- Provides training to participants and families regarding medications, medical conditions, safety, etc. as required and provides general health education as requested by program management.
- Communicates with participants’ physician/caregiver regarding health concerns and makes recommendations.
- Assists participants in obtaining medical services from other health services providers; schedules transportation as needed.
- Attends meetings with outside agencies, caregivers, and families. Networks with other agencies on behalf of program and participants.
- Mentors program team members and may help program leadership with orienting and training new team members in the areas of safety, participant/personal care, and nursing philosophy.
- Participates in the process of teaching, training, and mentoring student interns and volunteers working within the program area.
- Monitors safety of the work environment and employee safety practices.
- Attends weekly staff meetings, participant conference meetings, and in-service trainings.
- Acts in a manner consistent with agency values.
Qualifications
EDUCATION &/or EXPERIENCE:
- Must have a high school diploma or GED Equivalent and successful completion of a Licensed Vocational Nurse Certification program.
- One (1) year of acute care preferred. Experience working with a frail or elderly population or in a licensed health care facility or clinic setting, desired.
LICENSES OR CERTIFICATES:
- Must be a current Licensed Vocational Nurse by the California Board of Vocational Nursing.
- Current CPR and First Aid certification within six (6) months of hire.
- Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements, and that can be used for the applicable job functions noted above.
PRE-SCREENING REQUIREMENTS:
- Requires clearance of a DOJ and FBI criminal history background check, pre-employment physical, and PPD skin test.
- Requires Motor Vehicle Report verification.
JOB SKILLS:
- Knowledge of physical, social, and mental health programs operating within a licensed health care facility or clinic.
- Nursing knowledge and skills necessary to treat participants and manage complex nursing situations. Ability to provide care for the medically fragile, disabled, and older adult population in a Day Center setting.

- Proficiency in Microsoft Office, including Word, Excel, PowerPoint, Outlook, and relevant computer programs and software (e.g., electronic health record software, email, internet).
SUPERVISORY REQUIREMENTS:
- This position has no Supervisory requirements.
PHYSICAL REQUIREMENTS:
- Ability to sit at a desk and interact with a computer screen for extended periods of time.
- Hand and arm strength sufficient to operate a keyboard for several hours each day.
- Physical abilities sufficient to move between different staff work areas, communicate with staff and the public, operate a computer, produce reports, talk on the telephone, and travel to other alternative worksites.
- Working with medically fragile, disabled, and older adults in a variety of Day Center activities.
- Assisting participants with transfers, ambulation and group exercises, pushing occupied wheelchairs up and down ramps.
- Assisting participants to and from sitting positions and on and off large transportation van.
- Assist in set up and cleanup of activity supplies. Move tables, chairs, and activity equipment for different activities.
