Hummingbird Hospice is looking for a self-motivated Community Liaison to join our Team. We offer the opportunity to take on new challenges and a culture of teamwork that sets us apart.
The Community Liaison is primarily responsible for hospital, medical facility-based, and assisted living facility outreach to identify new referral sources and maintain strong relationships that will improve patient census and preference for hospice services. Liaisons will initiate communication with referral sources, families, and potential patients to help educate on hospice care and services.
Hummingbird Hospice is one of the top growing hospice providers of end-of-life care in San Antonio. We provide in-home hospice and palliative care services to those in need in and around our community.
Responsibilities
- Determines primary decision-makers and pro-actively builds positive, solid working relationships with members of the medical community and partners.
- Promotes and positions Hospice's patients care service through regular visits in a manner that illustrates and reflects the Hospice's quality of care and unique capabilities.
- Works to build the brand of the hospice as the preferred hospice provider in our service area to achieve and exceed agency goals for growth and census management.
- Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities, and other health care providers.
- May help Hospice in determining patient eligibility, based on their hospice knowledge and training.
- Conducts an initial meeting with the patient and family giving an overview of hospice service and philosophy and obtains consents for care.
Qualifications
- Minimum one (1) year of sales/marketing experience in healthcare.
- One (1) to two (2) years of experience in hospice/palliative marketing is strongly preferred.
- Skilled at establishing/maintaining working relationships with key staff, marketing contracts, and clinical staff to ensure a thorough understanding of Hospice services.
- Proven ability to generate and maintain relationships with new and existing accounts including physicians, healthcare professionals, and community agencies.
- The Covid-19 Health Care Staff Vaccination Interim Final Rule, also known as CMS IFR, requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid. Therefore, Care Hospice requires all colleagues, including new hires prior to their first day of employment, to submit appropriate documentation showing, i) completion of the full vaccination series against COVID-19 (i.e., two weeks past completing their complete COVID-19 vaccination series), or ii) approved exemption (medical contraindications or a sincerely held religious belief) through the formal vaccine exemption process from the COVID-19 vaccination requirement.
