Responsibilities include:
Telephonic triaging and assessment
Test result discussions and education
Administration of IV medications when needed
Chronic illness patient education
Surgical scheduling for multi-disciplinary providers
Performing office nurse visits when appropriate; ensuring chronically ill patients are comprehensively approached and communication with their primary cardiologist is conducted to avoid gaps in care.
Identifying patient needs and care coordination when necessary.
Working closely with physicians, APP’s, and care teams to ensure the highest level of communication and care is achieved.
Job Type: Full-time
Pay: $30.00 - $38.00 per hour
Expected hours: 40 per week
