Position Summary:
The Telehealth Coordinator is responsible for overseeing the implementation, management, and
evaluation of telehealth services within the Home Health Department. This role ensures the
effective delivery of telehealth programs to enhance patient care, improve outcomes, and
support the department’s goals. The Coordinator acts as the primary point of contact for
telehealth operations, bridging clinical staff, patients, and technology to optimize remote patient
monitoring and virtual care.
Key responsibilities include managing telehealth equipment, providing training and technical
support to staff and patients, monitoring program effectiveness, and ensuring compliance with
healthcare regulations and organizational policies. The Telehealth Coordinator collaborates with
interdisciplinary teams to integrate telehealth services into patient care plans and works to
identify opportunities for innovation and program growth. The Telehealth Coordinator serves as
part of the leadership team in the Home Health department and rotates through weekend and
holiday administrative duties.
This position requires a strong understanding of telehealth technologies, excellent
organizational and communication skills, and the ability to work collaboratively in a dynamic
healthcare environment. The Telehealth Coordinator is integral in advancing the mission of the
Home Health Department by expanding access to quality care and supporting patient
independence and well-being at home.
Primary Position Responsibilities:
1. Program Management and Operations: Coordinate and Provide Patient-Centered Care
Delivery via Telehealth Monitoring Program
2. Equipment / Technology Management, Training, and Support
3. Compliance and Innovation
4. Other duties as assigned to meet the needs of the agency.
Qualifications:
Minimum Education
Required: All hires after July 2012 are required to have a Bachelor’s Degree in
Nursing, OR if Associates Degree or Diploma, RN must provide proof of enrollment in a BSN
program prior to hire and complete the program within five years of hire date.
Minimum Years of Experience
Required: Two years of Med-Surg experience
Preferred: Two years of current home care experience. One to two years of management of high risk patients (cardiac, pulmonary, diabetic)
License, Registry or Certification
Required: Current CPR certification, NJ RN licensure, valid driver’s license
Preferred: Certification in specialty
Knowledge, skills and/or Abilities
Required: Excellent physical assessment skills, excellent communication skills, familiarity with Microsoft Office & Google suite.
Preferred: Knowledge of state and federal home health regulations
