What are the responsibilities and job description for the Wellness Coordinator position at Hunterdon Health?
Position Summary
The Wellness Coordinator will coordinate the overall integration of wellness services into Behavioral Health patient's care plan in order to assist patients in achieving mental health and wellness. The WC will partner with patients and providers to ensure fully-connected health and wellness care. The WC will serve as a patient advocate as well as an advocate for Hunterdon Health fitness and nutrition services. The WC is required to be knowledgeable to help answer questions and effectively guide patients through the wellness recommendations as suggested by the behavioral health team.
Primary Position Responsibilities
1. Coordinate patient's participation in wellness services by collaborating and actively communicating with service providers
2. Engage with patients throughout their participation in wellness programs to ensure their effective, timely participation
3. Assist in the development of program materials and resources
4. Monitor patient activity in wellness services and implement required intervention methods 5. Provide program orientation and assist with tranisiton to designated service areas 6. Serve as program point of contact in collaboration with provider network.
7. Assist in monitoring services to ensure high level of patient satisfaction.
Work Contact Group (Internal/External)
Behavioral health patients, Hunterdon Health staff and physicians. Hunterdon County local and
state agencies and organizations.

Qualifications
Minimum Education:
Required:
High School Diploma or Equivalent
Preferred:
Bachelor's Degree in Health science, health and wellness or commensurate experience
Minimum Years of Experience (Amount, Type and Variation):
Required:
Minimum 1 year experience working in a health or wellness setting.
Preferred:
Minimum 1 year experience working in a behavioral health and or care coordination.
License, Registry or Certification:
Required:
Valid NJ driver’s license
Preferred:
Willingness to obtain mental health training
Knowledge, Skills and/or Abilities:
Required:
Interest and ability to work with behavioral health patients; strong facilitation and problem solving skills; exceptional customer service skills; strong written and verbal communication skills; strong organizational skills; proficient computer skills. Ability to work independently and as part of a team and have reliable transportation
Preferred:
Experience in the area of health and wellness. Experience with crisis management. Experience with Excel spreadsheets, Microsoft Word, and other data management software programs, Google and
Electronic Medical Record (EMR).
