What are the responsibilities and job description for the Care Manager position at Huntingdon-Bedford-Fulton Area Agency on Aging?
Care Manager will be a registered nurse or possess a bachelor's degree in social work, psychology, gerontology or other related field and one year of experience in direct service to older persons. Responsible for seeking out and informing older residents of available Agency services, arranging care management services to consumers in their homes including developing care plans and evaluating effectiveness of services through meetings and counseling with consumers/families. Enter data and case notes into State-approved data base program. Assist with in-office I&R as scheduled, assist with guardianship consumers as requested. Knowledge of available aging & community support services, ability to plan, organize and prioritize daily duties. Excellent verbal, communication and computer skills, valid PA driver's license and access to transportation required. Good benefit package.
