Job Title : LPN - Per Diem

Shift :
24 hours minimum required within a 4-week period, with at least 2 of those shifts on weekends. Full 8-hour shifts.

Job Description :
- Provides direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Maintains collaborative relationships with other nursing staff, physicians, and ancillary staff.
- Participates in continuing education and in-service programs to maintain clinical expertise.
- Collects data relevant to the resident's physical and psychosocial condition and communicates findings to the RN and Interdisciplinary Team.
- Contributes to developing a plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
- Recommends revisions to the plan of care after evaluating resident outcomes and indicates the need for referrals.
- Collaborates with residents, families, and the Interdisciplinary Team to ensure all aspects of the resident's care are addressed.
- Participates in the mentoring / orientation of staff and maintains current knowledge and competency in nursing practice.
- Considers factors related to safety, cost, and impact in the delivery of care.
- Demonstrates knowledge and competency in the professional practice setting and anticipates occupational exposure to blood-borne pathogens.
