What are the responsibilities and job description for the Care Coordination Specialist FT- 1st shift position at Huntsville Hospital Health System?
Overview
The Coordination Specialist answers incoming telephone calls, emails, and faxed communications from local employers and employees needing assistance with personal health care navigation for primary care, specialty care, and procedures. Works with Communications Center and Corporate Care Team Lead to coordinate call intake process and flow. The specialist will develop relationships with provider scheduling staff for optimal results and customer service. Specialist will collect appropriate data using a customer relationship management system, report results, schedule appointments and services for clients/patients, obtains pre-certifications for necessary procedures and services and ensures accuracy of documentation prior to forwarding to appropriate party. Specialist must ensure superior customer satisfaction and increasing revenue by effectively routing referral patterns inside the health system. This position will report to the Senior Manager of Corporate Health Business Development.
Qualifications
Education: Associates or Bachelor Degree
Experience: Prior customer service, health services, health insurance, care management, scheduling, and/or related experience.
Additional Skills/Abilities •Knowledge of health care environment. •Computer literate with the ability to learn new software applications, develop reports, and review data. •Excellent communication and customer service skills required. •Attention to details and ability to handle multiple tasks simultaneously. •Ability to problem solve and communicate effectively.
