The Registered Nurse (RN) in Home Health plays a vital role in providing high-quality patient care in the comfort of their own homes. Your responsibilities will include:
- Coordinating comprehensive care plans with other healthcare professionals to ensure seamless continuity of patient care
- Conducting initial and ongoing assessments to develop personalized plans of care, in accordance with state-specific guidelines and physician orders
- Ensuring that patients meet home health eligibility and medical necessity requirements as defined by payer sources
- Maintaining accurate and timely documentation of patient care
To be successful in this role, you must possess:
- Current RN licensure in the state where you will be practicing
- Active CPR certification
