Job SummaryRetrieves charts from nursing units as needed. Accounts for all discharges and searches for missingcharts. Preps and scans medical records and performs QC and Validation on scanned records to assurehighest quality of legibility and accuracy. Interviews patients for birth certificate completion according toCenter for Vital Statistics. Initiates fetal death certificates and death certificate completion for all hospitaldeaths according to Center for Vital Statistics guidelines. Indexes and scans paper medical records intothe electronic health record, after removing documents that are not part of the permanent record.Prepares medical records for shipment to secure storage facility. Purges permanent records inaccordance with state and hospital regulations. Assigns documents for electronic signature. Analyzescharts for physician deficiencies and reports to HIM Department Supervisor. Performs other duties asassigned by supervisor.
