Overview
The Population Health Analyst works in partnership with patients, families, clinical staff, providers, physicians, advantage plans, specialists within and outside the network, other members of the Population Health Team and resource groups within the community. Coordinates proactive health initiatives for assigned population while ensuring care gaps are met. Facilitates coordination of care for patients with chronic conditions and manages effective care transitions for them through the continuum. Provides effective health coaching to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risks. Initiates and promotes the utilization of community resources. Facilitates a “shared goal model” within and across settings to achieve coordinated, high-quality, outcomes based care that is patient and family centered.
