What are the responsibilities and job description for the Care Coordinator position at Hurley Practice Management Services?
Shift will be 3:00pm-11:30pm Monday-Friday with rotating weekends, which equals out to 1 weekend a month and rotating holidays.
GENERAL SUMMARY:
Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Complies with all appropriate safety and infection control standards. Participates in quality assessment and continuous quality improvement activities. Performs all job duties and responsibilities in a courteous and customer-focused manner according to the Hurley Family Standards of Behavior.
SUPERVISION RECEIVED:
Works under the direction of a director/manager or designee who reviews work for conformance with medical center policies and procedures.
RESPONSIBILITIES AND DUTIES:
1. Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further understanding of significant social and emotional factors underlying patient health problems including interviewing patients and providing background summaries.
2. Complete the program eligibility assessment and consent form. Provide service plans to restore patient to optimum social and health adjustment within patient’s capacity.
3. Assists patient and family in discharge planning for home or other alternative living situations.
4. Process victim compensation applications and disability as needed.
5. Provide legal advocacy and safety coordination. Acts as liaison among the medical center, patients, facilities, courts and other community support agencies.
6. Demonstrates effective judgement and ability to understand, react effectively and treat (if appropriate) unique needs of patient age groups served.
7. Obtain basic needs items and problem solve on behalf of victims until stabilization.
8. Responsible for victim follow up and referrals. Will conduct home visits as necessary.
9. Performs other duties as required. Utilizes new improvements and/or technologies that relate to job assignment.
MINIMUM ENTRANCE REQUIREMENTS:
· Bachelors Degree
· Knowledge of community organization and social welfare.
· Ability to prepare and maintain accurate records and reports.
· Ability to establish and maintain harmonious relationships with physicians, patients, community support agencies, medical center staff and the general public.
