Job Summary
Ensures that the medical record is organized and complete. Responds to release of information requests in a timely manner.
Essential Job Functions:
- Perform chart analysis on each inpatient, observation, emergency and outpatient surgical encounters to ensure that all required documentation has been completed.
- Create deficiencies for any documents that have not been completed based on CMS guidelines and HRMC Rules and Regulations.
- Scan written documentation into the EMR.
- Complete birth certificates for all newborns and fetal death certificates. Enter information into the Electronic Vial Records Screening (EVRSS) for the Department of Vital Statistics. Prepare paternity paperwork as needed.
- Answer phone calls from HRMC employees, patients, insurance companies and other medical facilities.
- Promotes and maintains confidentiality when dealing with data related to patients, providers, co-workers and any other facility information.
- Provide education and assist patients and employees with utilizing the Patient Portal.
- Provide education to patients on the medical record process, options and requirements and direction on obtaining information relating to their healthcare.
Other Job Duties:
- Obtains documentation from other facilities as requested by coding staff.
- Participates in Quality, Risk and Utilization Management activities.
- Assist in the orientation and development of personnel and students.
- Continuously working on professional self-development. Ensures that they are knowledgeable about advances, changes and new regulations that may affect their job duties.
- Maintains the Master Patient Index.
- Maintains and organizes the medical records storeroom to promote timely retrieval of information.
- Purges medical records according to policy.
- Continuously reviewing and improving processes and workflow.
- Assist other departments and performs other duties as needed/able to.
- Ensures that the Mission, Vision and Values statement is utilized and applied in all duties.
- Complies with all HRMC policies and federal/state regulations.
- Always maintains a professional appearance and demeanor.
- Utilize interpretation resources as needed (via phone/computer or in-person)
Requirements
Qualifications:
- High school graduate or GED equivalent required. Medical office certificate preferred.
- At least one year of experience in a medical facility preferred.
Knowledge, Skills, and Abilities:
- Requires excellent verbal and written communications skills.
- Experience with Microsoft Office package including but not limited to Microsoft Word, Microsoft Excel and Microsoft Outlook.
