ESSENTIAL FUNCTIONS:
- Develops interdisciplinary care plan and other care management by coordinating information and care requirements with other care providers for specific group of patients.
- Maintains ongoing communications with referral sources and obtains accurate information on new patients.
- Receives calls from physicians, ED, outside referral agencies, hospitalists, etc. and facilitates transfers/admissions and bed placement.
- Serves as first-line contact for physicians and outside referring agencies.
- Collaborates with physicians and referring agencies in determining patients’ acute placement needs.
- Demonstrates a high degree of commitment to customer service excellence.
- Exhibits exemplary interpersonal relationship skills, along with concise written and verbal communications.
- Submits completed referral documentation within established time parameters.
- Investigates and notifies Care Management Director of any potential referral denials; communicate approved referral denials to appropriate personnel and document refusal appropriately.
- Follows regulatory requirements for HIPAA and EMTLA.
- Provides coverage for ED care management as assigned by supervisor.
- Accesses medical records as necessary.
- Documentation of all calls.
- Actively works with physicians on appropriate inpatient and outpatient status.
- Complies with hospital and legal requirements by fostering nursing practices that adhere to the philosophy, goals, and standards of care.
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
- Ability to develop patient plan of care.
- Extremely skilled in customer service.
- Demonstrated ability to communicate effectively.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
- Completion of an approved school of nursing program.
- One-year experience as a registered nurse.
- Case management/UR/Access Center experience preferred.
PREFERRED EDUCATION AND EXPERIENCE:
- BSN
- Care management and/or access center experience.
LICENSURE OR REGISTRATION REQUIRED:
- Current licensure as an RN in the State of Kansas.
