The Licensed Practical Nurse provides basic nursing services, supportive activities and assigned clerical functions for the Medical Services Staff and clients.Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.ESSENTIAL FUNCTIONS :
- Performs assessments and nursing functions, including : assessment of mental status, review of factors regarding level of functioning, psychotropic medications status and side effects.
- Communicates information to medical staff in a timely manner to assess the need for medication revisions.
- Assists with responding to phone calls from patients, families, and care agencies regarding client care issues, medication refills and problems. Uses basic nursing knowledge and / or medical provider consultation to provide counsel and recommendations for clients, family members and other Horizons personnel providing services to clients.
- Provides medication administration.
- Completes prior authorizations for medications for prescribers.
- Assists clients, caregivers and case managers in accessing medical care and medication assistance using patient assistance programs and community resources.
- Assists with the review / monitoring of lab work ordered by the medical provider, alerts medical provider of abnormal results and records findings in the medical record.
- Coordinates outpatient Clozaril treatment with clients, lab, and pharmacies and follows up with those clients, maintaining knowledge and changing criteria of the Clozaril protocol as it applies to each patient.
- Assists with appropriate referrals to therapists, case management services, crisis services, supported employment services and other referrals.
- Other duties as assigned.MINIMUM KNOWLEDGE AND SKILLS REQUIRED :
- Working knowledge of criteria used for diagnosis of mental disorders.
- Working knowledge of medical disease processes that may be impacted by mental disorders and their treatments
- Working knowledge of psychotropic medications, their indications, special risks, and side effects.PREFERRED KNOWLEDGE AND SKILLS REQUIRED :

- Graduate of an accredited LPN program.PREFERRED EDUCATION AND EXPERIENCE :
- One year or more of experience in inpatient or outpatient behavioral health nursing.REQUIRED LICENSE / REGISTRATION / CERTIFICATION :
- Current licensure as a LPN in the State of Kansas.
