Job Description
Principal Duties and Responsibilities
- Works and collaborates with team members and community partner agencies to help identify, evaluate, and assist potential and current clients
- Work with Case Managers to determine client needs and then work with assigned clients by assisting in building and maintaining stability while in permanent housing
- Collaborates with community partners to facilitate client care and assist with program operations
- Builds professional rapport and appropriate / ethical relationships with clients to benefit a client's self-sufficiency and housing stability
- Assists in facilitation of support groups in collaboration with case managers
- Coordinates activities and services for clients to help them achieve their goals
- Works with Property Management and helps remediate possible tenant issues
- Maintains client records and documents services in HMIS, per agency standards
- Prepares and updates reports as assigned
- Locates appropriate resources for clients and makes appropriate referrals
- Actively assists in updating / maintaining the program database to reflect current and accurate record keeping
- Regularly makes rounds at each assigned site monitoring client activities, securing facilities and ensuring safety of clients, and documenting pertinent information
