Department: Staunton Clinic
Work Hours: 8:30am-5pm. Primarily Monday through Friday daylight, extended hours as necessary to support operations.
Basic Function:
The community-based Lead Health Navigator/Wellness Nurse participates as a member of a multidisciplinary team to provide a specialized level of service to eligible service participants through the Behavioral Health Homes Plus Expansion (BHHPE) initiative. Responsibilities include assessment, client education in physical health, medication and illness management, medication management, and client specific individualized supports to commensurate with multidisciplinary team services; including reciprocal communication with physical health providers.
Qualifications:
Required:
Active RN license and two years related behavioral health experience; Valid Driver’s license; Knowledge of local physical health care provider network. Requires extensive knowledge of agency and community resources and services. Solid clinical knowledge of mental health, D&A, DSM-IV and ICD10 diagnostic categories. Demonstrates the ability to navigate, negotiate and advocate within multiple service systems. Ability to work independently and exercise sound judgment/ problem solving in the resolution of difficult situations and seeking supervision as needed. BLS certification. Successful completion of applicable clearances as outlined in Human Resources policy HR-106 prior to commencing employment.
