Medical Care Coordinator
Summary: The Care Coordinator is responsible for overseeing residents' medical care and wellness activities. This includes managing appointments, coordinating with healthcare providers, organizing enrichment activities, and maintaining accurate medical records. The role involves ensuring residents receive timely care and participate in meaningful activities, while coordinating communication between the care team, guardians, and case managers.
Key Responsibilities:
- Medical Appointments & Scheduling: Coordinate and schedule medical appointments for residents, ensuring timely follow-ups and adherence to medical schedules. Set up and arrange appropriate transportation for medical appointments and accompany residents.
- Liaison with Healthcare Providers: Facilitate communication between healthcare providers, the facility’s registered nurse, and the care team to ensure seamless coordination of resident care.
- Medical Records Management: Maintain organized, accurate, and up-to-date medical records, ensuring secure storage and easy access to important documents.
- Communication with Case Managers & Guardians: Provide regular updates to guardians and case managers on resident health status and care plans, addressing concerns promptly.
- Care Team Meetings: Organize and manage care team meetings, preparing agendas and documenting key updates, decisions, and action items.
- Resident Care Updates: Share important health and care updates with caregiving staff to ensure consistent and high-quality care for residents.
- Service Plans & Medication Management: Adjust individualized service and behavioral support plans. Oversee medication management, ensuring accurate record-keeping.
- Resident Activities & Enrichment: Organize resident enrichment activities and community outings to encourage social engagement and personal growth. Accompany residents to ensure their safety and participation.
- Transportation Coordination: Coordinate and set up transportation for both medical appointments and community outings.
Required Qualifications:
- At least 3 years of experience in healthcare coordination, case management, or a related field.
- Proficiency in Microsoft Word, Excel, and Google Docs.
- Valid driver’s license, proof of insurance, and reliable transportation.
- Strong organizational and communication skills.
- Ability to manage multiple tasks and collaborate with medical professionals and care teams.
- Must attend and accompany residents to medical appointments and community outings.
Preferred Qualifications:
- Associate’s degree in a relevant field (e.g., healthcare, case management).
- Experience in healthcare coordination or case management.
- Knowledge of medical terminology and documentation practices.
- Familiarity with electronic medication administration systems (EMAR).
Job Type: Part-time
Pay: $15.00 - $18.00 per hour
Expected hours: 24 – 32 per week
