Under the supervision of the Statewide Harm Reduction Manager performs duties relating to the HRC specific community case management and referral services as needed.
Tasks / Duties / Responsibilities
70% Client Services
- Provide HRC Case Management in designated county.
- Conduct Community outreach, provider education and client recruitment of individuals in need of Harm Reduction Services.
- Offer case management services to address obstacles while stabilizing at the HRC.
- Conduct assessments to determine clients' needs of supportive services.
- Refer clients to HRC staff and / or community agency staff to assist them with psychosocial supports (e.g., housing, transportation, employment).
- Follow-up with all referrals and clients to ensure all needs are being met.
15% Reporting / Monitoring
Establish, maintain and update client case records using approved agency forms and procedures. Ensure confidentiality of all records.Prepare monthly reports in accordance with agency policies and procedures.Maintain and update client information in required reporting system.10% Hyacinth representation / advocacy
Attend and participate in staff meetings and supervisory conferencesMaintain working relationships with other community providersObserve established administrative procedures.Provide written reports and maintain statistics as requiredRepresent the agency at community events, meeting and conferencesAttend and participate in staff training opportunities.Adhere to Hyacinth policies and procedures including client confidentiality provisions5% Training and certifications

Required Minimum Education : Bachelor's degree, can be replaced with appropriate experience
Required Years of Related Experience : 1-3 years, experience with Harm Reduction / Social services / community services
Required Knowledge, Skills and Abilities :
Active listening skillsFamiliarity with trauma informed care approachKnowledge of available services and community resourcesBi-lingual (English and Spanish) is a plusPeople Management Experience :

Must be knowledgeable about working with people with active substance use disorders.Must be culturally competent and sensitive to the varying socio-cultural factors that contribute to substance use.Must be able to work non-traditional hours and days including late nights and weekends in order to reach high risk populations.
