Description
Community Case Managers serve as a bridge between clients who have not been actively engaged in their own health care and staff in local clinics to ensure connection to medical care. CHWs conduct outreach to local clinics to find individuals who have been lost to care, and then conduct outreach in the broader community to find these individuals and re-connect them to medical care. CHWs also conduct outreach to underserved communities identified as at-risk for increased levels of HIV infection including - men and women actively engaged in substance use activities, incarcerated, undocumented, uninsured, homeless, or otherwise in denial of their risk of HIV infection.
Responsibilities including linking individuals to rapid HIV testing services and then determining whether the individual should be linked to PrEP (Pre-exposure Prophylaxis) or to medical care. For clients on PrEP, the CHW will request HIV and lab reports every three months. The CHW will provide supportive services including medical care and/or social service appointments.
Administrative duties:
Client Services · Provide Community Based Case Management in North Hudson · Conduct Community outreach, provider education and client recruitment of newly diagnosed and out of care clients in North Hudson · Refer clients to medical care and medical case management · Conduct assessments to determine clients’ supportive service needs · Attend medical and other social service appointments with clients. · Collaborate with primary care providers and MCM staff to identify newly diagnosed patients in need of Community based case management · Documentation of baseline viral load, CD4 count, AIDS diagnosis and antiretroviral prescriptions for 100% of clients.
Reporting /Monitoring:
Establish, maintain and update client case records using approved agency forms and procedures.
Ensure confidentiality of all records.
Prepare monthly reports in accordance with agency policies and procedures.
Maintain and update client information in eCOMPAS and CAREWare
Hyacinth representation/advocacy
Attend and participate in staff meetings and supervisory conferences
Maintain working relationships with other community providers
Observe established administrative procedures.
Provide written reports and maintain statistics as required
Represent the agency at community events, meeting and conferences
Attend and participate in staff training opportunities.
Adhere to Hyacinth policies and procedures including client confidentiality provisions
Training and certifications · Attend Hudson County TGA Planning Council · Participate on Hudson County TGA EHE Task Force
Requirements
Required Minimum Education: Associate’s Degree Other: NoneRequired Years of Related Experience: 1-3 years
- Social services/community services
- A minimum of two years experience in the HIV field.
Required Knowledge, Skills, and Abilities:(Include any required computer skills, certifications, licenses, languages, etc.)
- HIV treatment education
- Community outreach
- Fluency in Spanish is required
- Demonstrated ability to work with people from diverse backgrounds, especially with members of LGBTQ and interests
- Access to a currently registered car and a valid driver's license.
