Position Description

Under the supervision of the Unit Manager, the LPN assumes responsibility and accountability for a group of residents/patients for a shift of duty. Nursing care is provided through assessment, implementations, and evaluation of the plan of care. The LPN adheres to the standards of care for the area, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activity performance by nursing assistants. Follows all facility policies and procedures. Performs duties as defined by the State Nurse Practice Act.

Job Duties

  • Gives/receives the nursing report upon reporting in and ending shift duty hours.
  • Makes frequent rounds to note resident/patient conditions and to ensure nursing personnel are performing their work assignments in accordance with acceptable nursing standards.
  • Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post-acute care facility.
  • Ensures assigned work area (i.e. nurse station, med. Cart, medication room, etc.), resident/patient care rooms and treatment areas are maintained in a clean and sanitary manner. Monitors medication amounts and orders from pharmacy as needed.
  • Must possess a positive attitude and understand the importance of teamwork.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Handles on-call responsibilities as required.
  • Counts all controlled drugs with other nurse at shift change and notifies the DON / Unit Manager of all drug and narcotic discrepancies noted on your shift.
  • Keeps medication room and medication cart locked according to regulations. Responsible for security of keys.
  • Keeps medication room and medication cart clean and orderly.
  • Participates in the patient Plan of Care process as assigned.
  • Able to communicate positively and effectively with Resident family members and representatives.
  • Complies with laws and regulations applicable to position and acts in accordance with North Central Care Center policies.
  • Treats all residents, visitors, and staff with courtesy.
  • Follow facility dress and hygiene policies.
  • Follows all Safety rules and regulations and reports violations immediately.
  • Knows and follows the Resident Bill of Rights.
  • Knows and follows HIPAA regulations.
  • Reports all suspected violation of company’s HIPAA policies or procedures to Unit Manager or DON.
  • Provides and delivers nursing duties as assigned in a timely manner.
  • Education: Graduate of an accredited school of professional nursing.
  • Certifications & Licenses: Possesses a current license to practice in Washington State as an LPN. Must possess a current CPR Certification, which must remain current during employment. Stays current on Continuing Education credits for licensure.
  • Experience: One year of experience in long term care setting is preferred.
  • Skills, Knowledge and Abilities: Ability to manage and supervise nursing staff. Ability to make independent decisions. Ability to read, write, speak and understand English. Meets all health requirements, as required by law. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other center staff. Demonstrates basic computer knowledge (Microsoft Office) and ability with an aptitude to learn company applications.
