Resident Care Coordinator
Supervised by: Director of Nursing
Job Overview
This position provides clerical support and recordkeeping for the medical aspects of resident
care at the facility. Under the general supervision of the Director of Nursing, the Ward Clerk is
expected to work independently in the processing and recording of forms, records, and vital
statistics. This position performs duties that maintain order and efficiency of the nursing station,
meet facility needs and comply with State, Federal and local regulations.
Basic Qualification
Education/Training: Knowledge of medical terminology. Good spelling and telephone etiquette.
Knowledge of MDS assessment, and clinical software. High school diploma or equivalent.
Skills: Must have good spelling and telephone etiquette. Must be able to work independently.
Typing 40 wpm preferred.
Experience: 1-2 years’ experience in the medical field. Previous Ward Clerk experience
preferred.
Standard Requirements

3. Support, coordinate with, and implement specific procedures and programs for:
a. Safety, including universal precautions and safe work practices, established
fire/safety/disaster plans, risk management and security, report and/or
correcting unsafe working conditions, equipment repair and maintenance needs
b. Confidentiality of all data, including resident, employee, and operations data.
4. Support and participate in common teamwork:
a. Cooperates and works together with all co-workers; plan and completes job
duties with minimal supervisory direction, including appropriate judgment.
b. Uses tactful, appropriate communications in sensitive and emotional situations.
c. Follow up as appropriate with supervisor, co-workers or residents regarding
reported complaints, problems, and concerns.
d. Promotes customer service with residents, family members and guests.
e. Adheres to requirements for in-service training; attendance, uniform,
department, and dress code policies, including personal hygiene, and completes
other work duties as assigned.
Essential Duties
1. Transcribes physicians’ orders, notifies other facility departments regarding orders
affecting them, and notifies pharmacy.
2. Reviews medical records to maintain quality assurance and notifies Unit Supervisor of
any discrepancies found.
3. Maintains confidentiality of all resident information.
4. Performs as receptionist for unit, which includes answering phones and door, delivers
messages to staff and residents, delivers mail/flowers to residents, greets, and directs
visitors to residents’ rooms.
5. Maintains a current listing of frequently used phone numbers of hospitals, clinics,
specialists, and transportation companies for staff use.
6. Completes necessary chart paperwork and transportation for
admission/discharges/transfers; obtains photograph for medical record of all residents
upon admission.
7. Coordinates clinic/hospital appointments for residents, which includes notification of
family members, arrangements for appropriate paperwork to be sent, and
transportation there and back.
8. Maintains oxygen supplies for the facility
9. Maintains records of all labs for residents, notifies nurses regarding labs required each
week and transports lab specimens as needed.
10. Updates and maintains resident information in facility computer program, which
includes: MDS, diagnostic list, special needs listing, resident roster, census and
conditions and admission information, prints out reports as needed.
11. Maintains and updates wandering listing for facility.
12. Files dictations, labs, x-ray reports in chart.
13. Notifies pharmacy of any changes needed prior to med sheets/treatment
sheets/behavior sheets are due to come to the facility monthly; once arrived, processes
these sheets to appropriate books for nursing staff’s use.
14. Participates in appropriate in-service training programs and fire safety/disaster
preparedness drills.
15. Follows established procedures in infection control and use of equipment and supplies.
16. Charts in nursing notes, record vital signs, weights as needed, record medical
information in accordance with established documentation policies and procedures.
17. Updates and prints our resident information reports.
18. Records resident identification data on designated med records and ID cards.
19. Copies medical forms and resident chart information as needed. Notifies MD’s of visit
requirements and when they need to see residents.
20. Coordinates yearly flu vaccines for residents.
21. Performs other duties as assigned.
