Landmark Care Center and Rehab is Yakima’s premiere long term care and rehabilitation facility. In this Management role, you will be responsible for the residents on our Long-Term Care Unit. You will manage the nurses, medication aids and CNAs caring for these residents. This leader also oversees the admission and discharge process. You will meet daily with our interdisciplinary team for updates and coordination of care. Tasks include charting in Point Click Care, managing care plans, processing orders, and assisting with medications and treatments as needed. This nurse also manages resident appointments, Hospice referrals, communication with resident families and Providers and other details of daily resident care.

We offer our residents a fully equipped rehabilitation gym with therapists from all disciplines that work with your patients on strengthening, as well as a restorative program for our long-term care residents. Detailed Requirements
1. Support the Missions, Values, and Vision of Hyatt Family Facilities and the facility.
2. Protect Resident Rights and ensure an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment.
3. Support, coordinate with, and implement specific procedures and programs for:
a. Safety, including universal precautions and safe work practices, established fire/safety/ disaster plans, risk management and security, report and/or correcting unsafe working conditions, equipment repair and maintenance needs
b. Confidentiality of all data, including resident, employee, and operations data.
4. Support and participate in common teamwork:
a. Cooperates and works together with all co-workers; plans and completes job duties with minimal supervisory direction, including appropriate judgment.
b. Uses tactful, appropriate communications in sensitive and emotional situations.
c. Follow up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems, and concerns.
d. Promotes customer service with residents, family members and guests.
e. Adheres to requirements for in-service training; attendance, uniform, department, and dress code policies, including personal hygiene, and completes other work duties as assigned.
Essential Duties
1. Supervise nursing activities in the facility during your shift.
2. Assist in developing and maintaining nursing objectives and standards.
3. Assess patients exhibiting signs/symptoms of condition change.
4. Ensure that each resident’s right to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights is followed by all nursing personnel.
5. Assure that residents have a clean, safe, orderly, and comfortable environment.
6. Assist in the training of department personnel.
7. Meet with residents or family members as necessary.
8. Perform administrative requirements such as completing necessary forms, reports, etc., and submitting to DNS as required.
9. Completes resident assessment for admit, discharge, and change of condition. Assist in evaluating and assessing medical, social, psychological, spiritual, restorative, and rehabilitative care plans, as well as instructions in self-help.
10. Maintain confidentiality of all pertinent resident care information to assure that residents’ rights are protected.
11. Assure that a stock level of medications, medical supplies, equipment, etc., is always maintained at adequate levels.
12. Consult with the residents’ physician in planning resident care, treatment, rehabilitation, etc., and periodically review and update resident care plans as necessary.
13. Assist in developing nursing care plans for individual residents.
14. Notify the residents attending physician, as well as the resident’s legal guardian/ representative, when the resident becomes critically ill, injured, etc., as outlined within our established policies.
15. Evaluate emergencies and determine emergency measures to be taken.
16. Make independent decisions regarding nursing care.
17. Notify physician of accidents and complete incident report forms.
18. Assist in establishing the administration, procedures, and control of medication.
19. Provide and update documentation for physician rounds. Make rounds with the attending physician as necessary.
20. Assist the nursing personnel in performing nursing procedures as necessary.
21. Additional duties as deemed necessary and appropriate by the DNS.
Other Duties
1. Performs tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.
2. Meet on-going requirements to maintain current Nursing license.
Physical Demands of the Job:
1. Frequent tasks involve walking throughout the shift. Distances typically less than 200 feet with occasional walking greater distance, walking is on even surfaces.
2. Occasional sitting during duties, the majority of which will be at the nurse’s station.
3. Occasional standing does not need to be static.
4. Occasional squatting and/or bending, necessary mostly during medication pass or residents’ assessment.
5. Lifting up to 10 lbs. frequently, 11 to 25 lbs. occasionally, 26 to 50 lbs. seldom and lifting greater than 50 lbs. is not required.
6. Repetitive movements require following proper body mechanics. Notify supervisor of any concerns. Fine repetitive motor skills will be used frequently; gross repetitive motor skills will be used occasionally.
7. Occasionally push/pull up to 25 pounds of force.
8. Occasional twisting, seldom climb or use stairs; never crawl.
9. Frequently reach from shoulder height to knees; seldom reach above shoulders or below knees.
Conditions of Environment
1. Controlled temperature, primarily indoor, well lit, maintained flooring, smoke free and occasional odors that dissipate. The noise level is maintained quiet to moderate.
- Minimum 3 years nursing experience and 1 year of management experience required, preferably in LTC
- BLS Certification/CPR card required
- Good communication skills
- Must be able to push/pull med cart when necessary
