Landmark Care Center and Rehab is Yakimas premiere long term care and rehabilitation facility. In this role, you will be responsible for Admissions, charting in Point Click Care and assisting as needed.

We offer our residents a fully equipped rehabilitation gym with therapists from all disciplines that work with your patients on strengthening, as well as a restorative program for our long-term care residents. Your role will allow you to collaborate to develop and implement individualized care plans.
As a leader you will manage the NACs assigned to your shift as they assist with resident care.
Landmark Care Center is conveniently located near the highway in a modern and well-cared for facility. We are located at 710 N. 39th Avenue in Yakima.

- RN or LPN License with at least one year of experience preferably in LTC
- BLS Certification
