Essential Job Function
• Provides speech therapy services to residents and initiates referrals when appropriate and in accordance with the Therapy Department Procedures.
• Conducts screening of residents at regular intervals to determine need for intervention / treatment.
• Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
• Interprets and communicates evaluation findings to residents, families, and team members.
• Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
• Coordinate’s treatment plan with resident, family and other team members.
• Implements and conducts treatment as outlined in treatment plan.
• Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
• Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
• Documents results of resident’s assessment, treatment, follow-up, and termination of services.
• Adapts treatment to meet the individual needs of the resident.
• Provides, recommends, fabricates adaptive devices, or other equipment. Trains resident, family, and appropriate staff in the use or application of orthotics and prosthetics.
• Instructs resident, family, and staff in appropriate speech-language procedures or home program to allow carry over of therapeutic remediation and goals in daily activities.
• Participates in discharge planning.
• Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using pre-determined criteria.
• Participates in resident, family, and staff education.
• Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
• Adheres to Speech Therapy Department procedures regarding documentation of speech therapy services.
• Provides in-service education to the members of the resident’s care team and education to the community.
• Provides treatment in accordance with established standards of practice, department procedures, and productivity standards.
• Consistently demonstrates sound judgement in the evaluation, planning, implementation, and follow-up of resident therapy programs.
• Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
• Assists with and participates in Quality Assurance Program as required.
• Maintains a current knowledge of law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are incompliance with applicable regulations.
• Attends and participates in scheduled trainings, educational classes and meetings required for requested for position.
• Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions, and OSHA standards for the healthcare personnel.
• Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/ appropriate personnel immediately.
• Responds to and acts appropriately in emergency or disaster situations.
• Adheres to high standards of cleanliness, grooming, hygiene and dress code.
• Adheres to all policies of company including job description, certification requirements, Mission Statement, HIPPA, confidentiality standards and residents rights.
• Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and is flexible in carrying out assignments.
• Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control
• Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
• Performs all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization

a. Safety, including universal precautions and safe work practices, established fire/safety/ disaster plans, risk management and security, report and/or correcting unsafe working conditions, equipment repair and maintenance needs
b. Confidentiality of all data, including resident, employee, and operations data.

a. Cooperates and works together with all co-workers; plan and completes job duties with minimal supervisory direction, including appropriate judgment.

d. Promotes customer service with residents, family members and guests.
e. Adheres to requirements for in-service training; attendance, uniform, department and dress code policies, including personal hygiene, and completes other work duties as assigned
Physical Demands of the Job:
1. Frequent tasks involving walking throughout the shift. Distances typically less than 200 feet with occasional walking greater distance, walking is on even surfaces.
2. Occasional sitting during duties, the majority of which will be in the Therapy Gym.
3. Occasional standing does not need to be static.
4. Occasional squatting and/or bending,
5. Lifting up to 10 lbs. frequently, 11 to 25 lbs. occasionally, 26 to 50 lbs. seldom and lifting greater than 50 lbs. is not required.
6. Repetitive movements requires following proper body mechanics. Notify supervisor of any concerns. Fine repetitive motor skills will be used frequently; gross repetitive motor skills will be used occasionally.
7. Occasionally push/pull up to 25 pounds of force.
8. Occasional twisting, seldom climb or use stairs; never crawl.
9. Frequently reach from shoulder height to knees; seldom reach above shoulders or below knees.
10. During the Covid-19 outbreak, N-95 masks are to be worn during the duration of your shift.
11. When facility is not in an active outbreak, staff are to wear surgical masks, KN-90/95s unless on observation which N-95 are still mandated.
Conditions of Environment
1. Controlled temperature, primarily indoor, well lit, maintained flooring, smoke free and occasional odors that dissipate. Noise level is maintained quiet to moderate.
2. Potential exposure of Covid-19 and mutated strains.
Qualifications
Master’s Degree in Speech Language Pathology from an accredited program.
Certificate of Clinical Competency or eligibility for a Clinical Fellowship Year.
Assumes responsibility for ongoing continuing education and professional development.
Salary : $50 - $60
