Position Overview:
Nurse Practitioner - Stroke Navigator, under the guidance of the Stroke Program Coordinator, serves as a key nursing consultant and leader, bridging the communication between medical staff, nursing staff, patients' families, and support services. The role involves coordinating multidisciplinary care based on evidence-based practices, delivering stroke education, overseeing patient care, and providing post-discharge follow-up. Additionally, the Nurse Navigator collaborates with key stakeholders in the Stroke Program to ensure certification standards are met, including data review and performance improvement initiatives. The patient experience is integrated into all aspects of care delivery.
Qualifications:
- Active California RN license
- State of California Nurse Practitioner licensure (including a furnishing number)
- DEA furnishing certificate
- BLS (Basic Life Support)
- ACLS (Advanced Cardiovascular Life Support)
- NIHSS certification within 3 months of hire/transfer
- ASLS certification within 3 months of hire/transfer
- ENLS certification within 6 months of hire/transfer
- CNRN or NVRN certification
- Masters Degree required
- SCRN certification preferred
We are open to considering Nurse Practitioners with limited NP experience, including recent graduates, if they have strong ICU, neuro, or emergency department nursing experience.
Compensation:
The pay range for this position is $59.24 - $94.79 per hour. Individual wages are determined based on various factors, including qualifications and experience.
