Summary/Objective
This position will provide direct services – including Day & Vocational Habilitation and Non-Medical Transportation Services – for adults with intellectual and developmental disabilities and behavioral health challenges. Programs are community and/or center based – including outings with the people we support.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Provide individualized support to participate in community activities and build community membership consistent with the individual’s interests, preferences, goals, and outcomes as outlined in the Individual Service Plan (ISP).
Help develop, restore, or retain the skills necessary for the individual to create and maintain a meaningful social life that offers opportunities for personal growth, independence, and natural supports through community involvement, participation, and relationships.
Provide personal care, including ambulation and transfer, support and supervision in personal hygiene, eating, communication, mobility, toileting, and dressing, to ensure an individual's ability to experience and participate in community living is available.
Use evidence-based techniques, as available, to implement behavioral support strategies and provide assistance in the use of communication and mobility devices as needed.
Support individuals in daily functioning to improve self-management of the negative emotional symptoms that interfere with a person's daily functioning.
Support opportunities that increase problem-solving skills to maximize an individual's ability to participate in integrated community activities independently or with natural supports.
Assist the individual in developing self-advocacy skills; to exercise his or her civil rights; to exercise control and responsibility for the services he or she receives; and acquire skills that enable him or her to become more independent, productive, and integrated within the community.
Identify strategies that assist the individual in performing activities that result in increasing his or her social integration with other persons.
Assist the individual with self-medication or provision of medication administration per DODD Medication Administration regulations.
Assure that proper procedures are followed regarding each individual and their inherent rights.
Maintain safe and healthy conditions in the building and vehicles and promptly report maintenance needs to the Operations Manager.
Provide transportation to individuals, as required or assigned.
Report incidents that adversely effect the health and safety of individual per DODD requirements.
Timely completion of all documentation.
Perform all other duties as assigned.

This job operates in the community and in centers.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee is required to lift, bend, push, pull up to 50lbs.
Position Type and Expected Hours of Work
This is a full-time position, and hours of work and days are generally Monday through Friday, 7:00 a.m. to 3:30 p.m. Schedule may vary based upon transportation route. Occasional evening and weekend work may be required as job duties demand.
Travel
Travel is primarily local during the business day.
Required Education and Experience
High School Diploma or equivalent.
Valid Driver’s License and Insurable Driving Record.
Preferred Education and Experience
Two years’ experience working with people with intellectual and developmental disabilities.
Additional Eligibility Qualifications
Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with consumers and other staff.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
Must possess the ability to make independent decisions when circumstances warrant such action.
