Summary/Objective
Primary job responsibilities involve supervision of initial needs assessments and reassessments, intake services, in-patient, community, and alignment file referrals, resources, linkage, and assignment of a care coordinator to support youth and families/caregivers in the OhioRISE plan in achieving their health and outcomes goals. Outreach Supervisors are primarily remote and may be able to perform some tasks in the community. Outreach Supervisors will work with individuals, parents/guardians, Boundless staff, community members, and other service and support providers via face-to-face engagement, telephone, video conferencing, and electronic communication. Day-to-day activities are varied based on the needs of the initial referrals received for OhioRISE members and individuals and families / caregivers on the team’s caseload. Outreach Supervisors are responsible for ensuring their team members are trained and follow all required standards, guidelines, policies, and regulations. Operations Supervisors are responsible for ensuring their team members meet all expected timelines and outcomes, including completing initial assessments, outreach, documentation, and reporting. Operations Supervisor will supervise all day-to-day operations; communications with guardians when a problem arises, other relative concerns, crisis, provider questions and concerns.
Essential Functions
- Supervise and ensure ongoing assessment of needs of individuals and family/ caregivers utilizing assessment and screening tools, including completion, and updating of the Ohio Brief or Comprehensive CANS (Child and Adolescent Needs & Strengths) assessments, as required.
- Supervise and ensure all initial and ongoing outreach, contacts, and service coordination with individuals and family/ caregivers are completed within required timelines.
- Participate in and monitor the development, review, and updating of monthly staff audits according to required timelines.
- Supervise and ensure referrals and linkages to appropriate services and supports, including natural supports, while in the outreach division and making sure initial contacts are completed for a total of 3 per month.
- Supervise and monitor discharge planning activities for youth declining OhioRISE, assess out of the appropriate tier level and when staff cannot reach the members, receiving no responses.
- Supervise and monitor transition activities for youth transitioning between the facility and community-based settings.
- Monitor the transition, as appropriate, of youth and families/ caregivers out of the OhioRISE program.
- Provide on-call availability and support for youth and families/ caregivers outside regular business hours, as needed.
- Conduct ongoing community resource development to identify formal and informal resources in the communities served, initially and on an ongoing basis, paying particular attention to the availability of culturally responsive resources. Educate team members regarding available resources and update databases to share resource information.
- Participate in team and community meetings regarding necessary resource development specific to the communities served.
- Supervise and ensure the completion of intake necessary forms and documentation, including consents and Release of Information forms.
- Establish and maintain positive relationships with the local system of care partners such as the children’s services system, Title IV-E agencies, boards of developmental disabilities, local corrections and court systems, schools, family, and children first councils, managed care organizations, providers of behavioral health services, and primary care providers.
- Supervise and monitor completion of all documentation, including gathering and reporting health and outcomes data.
- Respond to and be available for clients and families in crisis, as needed, including referencing the crisis plans and facilitating the appropriate linkage to de-escalate the crisis.
- Ensure team members meet the organization's engagement, documentation, and contract requirements.
- Provide excellent customer service and communication coordination between team members, individuals, families/ caregivers, community members, and collaborating organizations.
- Create a collaborative and engaged team environment where passionate team members feel educated and supported to perform their daily activities.
- Perform hiring, evaluation, coaching, and other personnel-related activities in accordance with the organizational policies and procedures.
- Ensure team participation in all quality improvement activities, as applicable.
- Ensure team members complete all required training according to established timelines.
- Ensure team members are trained on and accurately reporting any reportable incidents according to organizational policies and procedures and regulatory body rules.
- All other assigned duties.
- Adheres to company policies and procedures in addition to regulations, laws and other requirements from applicable governing bodies, certification, licensure and/or accreditation entities, etc.
- This job consistently works from home or in the community, providing services while working remotely, in schools, client homes, and other community settings. This role routinely uses standard office equipment such as computers, phones, and photocopiers.

Supervisory Duties
This position will supervise a team of Program Coordinators and Health Data Specialists.
Education, Skills, Experience and Requirements
Required:
- Bachelor’s degree in psychology, human services, social work, or related field.
- Three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth and their families or caregivers.
- Experience in one or more of the following areas of expertise:
- Family systems
- Community systems and resources
- Case management
- Child and family counseling or therapy
- Child protection
- Child development
- Training in cultural competency or the ability to complete such within 90 days (about 3 months) of hire.
- One year of supervisory or management experience.
- Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles
- No more than 5 points on driving record.
- Valid car insurance.
- Current High-Fidelity Wrap-Around and CANS Assessor training or the ability to complete such within 90 days (about 3 months) of hire.
Preferred:
- Master’s degree preferred.
· Current Ohio mental health professional license including LSW, LPC, LMFT, LISW, LPCC, LIMFT, PSY, LIMFT-S, LISW-S, or LPCC-S.

Work Hours: This is a full-time position, and hours of work and days are Monday through Friday, 8:30 am to 5:00 pm. Evening and weekend work may be required as job duties demand based on needing to link an individual or family in crisis to the appropriate resources or perform initial outreach to a crisis referral.
Travel Requirements: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected occasionally.
