Description
The Care Coordinator will support the clinic and physician care team by providing administrative and patient care services.
This is a full-time position working M-F from 7 : 30 am - 4 : 00 pm out of our Edina and Oakdale locations.
Preventive Cardiology Clinic is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
Essential Functions :
- Provide communication and liaison between providers and patients
- Screen all patient calls and respond to patient in a timely manner.
- Accurately and efficiently schedule new and existing patients' clinical appointments, imaging, consults, and any other tests ordered by physician. Process orders accordingly.
- Verify and document insurance information as defined by current business practices
- Maintain a working knowledge of health care plan requirements and health plan networks.
- Contact insurance companies for coverage of benefits and retrieval of pre-certification for diagnostic tests.
- Coordinate the transmission and acquisition of patient records
- Maintain Physician schedule, communicating changes appropriately
