Position Overview: The Case Managers work with older adults and people with disabilities to ensure they have access to services to allow them to live as independently as possible. As a Case Manager you will meet with older adults and people with disabilities to assess their needs, educate them about available services and assist them in securing the services they need.
Position Responsibilities: Conduct in-home assessments of the consumer's overall situation to determine needs to create and implement person-centered care plans including identifying, coordinating, authorizing, securing and monitoring community based services on an ongoing basis. Modify and adjust care plans as the consumer's needs or situation changes.
