Employment Type: Part time Shift: Day Shift Description: Position Purpose: The Anticoagulation Clinical Staff are responsible for the Anticoagulation Clinic Service. The pharmacist assists prescribers in caring for patients on anticoagulation therapy to achieve desired outcomes. What you will Do: Provides oversight and guidance for the Anticoagulation Clinic Service. Performs drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients. Provides appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocol and Chest Guidelines Educates patients and families regarding anticoagulation therapy. Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes in Coagclinic® and Webcare. Communicates effectively with prescribers referring patients to the Anticoagulation Clinic. Maintains competency in anticoagulation care. Serves as an active patient advocate regarding anticoagulation therapy Uses a team approach to working with staff and others Hours/Schedule: PRN; as needed. Minimum Qualifications: At a minimum, a BS in Pharmacy from an accredited college of pharmacy. PharmD preferred. Iowa Pharmacist Licensure, either by examination or by reciprocity. Demonstrated competence in providing direct patient care. Previous anticoagulation management experience preferred. Self-motivated, detail-oriented individual able to work with minimum supervision. Good communication skills. Leadership skills. Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
