This includes those who may have Medicare, Medicaid, HMO or private insurance to cover their stay at various units. Discharge planning is coordinated with physicians, nursing, patient and significant others who have an ongoing caring relationship with the patient. Is knowledgeable of criteria for Medicare, Medicaid coverage and that of HMO or private insurers. Responds to referrals same working day, referrals will be sent to HHC agencies, DME companies & SNFs. Provides IMs & patient choice, documents discharge planning progress notes to provide clear / concise record of discharge panning service assessments and interventions. Accepts delegations from Case Managers for succinct care coordination.CUSTOMER SERVICE RESPONSIBILITIES : Performs duties and conducts interpersonal relationships in a manner designed to project a positive image of the department and the hospital. Works with supervisor / manager and other members of section or department to promote a harmonious work environment. Ensures that all contacts with patients, the public, physicians and other hospital personnel are carried out in a friendly, courteous, helpful and considerate manner. Answers telephones in a prompt and courteous manner. Displays concern and provides assistance or explains procedures as appropriate to callers or in face-to-face situations. Dedicated to meeting the expectations and requirements of internal and external customers.
