A chronic care management medical assistant is responsible for:
- Communicates with patients by telephone including: answering patient questions and concerns according to the physician’s protocol. Appropriately identifies situations that may be serious and require the immediate notification of the physician.
- Help manage medications associated with patient’s chronic care condition. Will also assist in verifying and updating medication information, discussing medication refills; and ensuring patients understand their prescriptions.
- Reminding patient of upcoming appointments and educating them on the importance of preventive measures.
- Educating patients on managing chronic conditions and adhering to treatment plans
- Regularly monitor/follow up with patients on their chronic care conditions
- Prepares medical records for appointments including; obtaining lab, x-ray, past endoscopy, and pathology reports; obtaining medical records from other facilities as needed; obtaining any other required information if not readily available in the medical record.
- After the physician has reviewed labs, follows through with his orders as well as calls patients with orders as needed.
- Demonstrates the ability to recognize normal, abnormal and “panic” lab values, and communicate these results to the physician.
- Maintains current knowledge of Medicare’s Evaluation and Management Documentation Guidelines, and Local Medical Review Policies regarding medical necessity procedures.
- Functions with the legal limit of the practice
- Performs all duties and functions in strict compliance with the center’s Corporate Compliance Plan, and all applicable federal, state, local, and regulatory agency requirements.
- Performs other duties as needed or directed by the physician, Nursing Supervisor, or Business Manager.
Experience
- Previous experience as a Medical Assistant or in a similar role is preferred but not mandatory.
- Proficiency in using electronic health record (EHR) systems for documentation and patient management is desirable.
- A solid grasp of medical terminology to facilitate effective communication within the healthcare team.
