Description:

- Counsels consumers while providing supervision, training and assistance in accomplishing activities of daily living and independent living skills.
- Physically assist/coach consumers with toileting and other personal hygiene needs on a daily basis.
- Maintains up-to-date program notes, outcome notes and billing for each consumer assigned on a daily basis.
- As needed, transports consumers to various activities, including volunteer activities.
- Transports consumers back and forth to program on a daily basis.
- Maintain agency vehicle to ensure safety. Submits all gas receipts to Coordinator.
- Assists consumers in and out of the van during activities.
- Ensures that fire safety needs are met, including participation in quarterly fire drills or as needed.
- Ensures the safety of consumers at all times.
- Ensures the facility is clean and notifies maintenance staff and/or supervisor of any issues.
- Attends mandatory training and/or meetings as scheduled.
- Maintains up to date all necessary first aid and CPR certifications.
- All other duties as assigned by the supervisor.

Skills and Abilities:
- High School diploma or GED
- Valid NYS driver’s license
- 6 months experience working with community human service providers and working with Developmentally Disabled population
- Bilingual preferred, not required
- Basic cooking skills
- A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO
Physical Demands:
The position does require occasional standing, squatting, lifting of up to approximately 40 lbs.
