Overview
We are seeking a highly motivated and experienced RN Case Manager to join our team in Columbus, OH. The RN Case Manager will be responsible for coordinating and managing patient care throughout the healthcare continuum. The ideal candidate will have excellent communication and organizational skills, as well as a strong clinical background.
Responsibilities
- Assess patient needs and develop individualized care plans
- Collaborate with healthcare team members to ensure quality patient care
- Coordinate and manage patient care throughout the healthcare continuum
- Provide education to patients and families regarding their healthcare needs
- Monitor and evaluate patient progress and adjust care plans as needed
- Ensure compliance with regulatory and accreditation standards

- Maintain accurate and timely documentation
Requirements
- Current RN license in the state of Ohio
- Bachelor's degree in nursing preferred
- Minimum of 3 years of clinical experience
- Experience in case management preferred
