What are the responsibilities and job description for the Family & Community Based Services Specialist position at ICAN Inc?
A Family and Community Based Services Specialist oversees all activities related to the quality assurance and implementation of home and community based services and care management services for youth and adults enrolled in Health Home Services. This position adheres to and promotes the philosophy and missions of the company by performing the following duties and responsibilities.
Duties & Responsibilities:
- Conducts outreach to community partners and Managed Care Organizations to manage community-based services.
- Works as the liaison between ICAN and care management agencies to ensure that services are provided to clients as deemed appropriate and authorized; on an as needed basis.
- Links clients to community service providers to perform approved Medicaid managed care services.
- Collaborates with the Director of IPA Operations to ensure the quality of community based service provider services in our region.
- Works in conjunction with care management to ensure continuity of care.
- Assists care management staff with compliance in electronic health records utilized by designated health homes.
- Manages all components of community-based services within the ICAN electronic health record.
- Assists / serves as support to other children’s Medicaid billable services.
- Works with the finance department to ensure billing quality.
- Acts as a liaison between client and family, agency personnel and other service providers as necessary and appropriate.
- Ensures frequent collaboration with managed care organizations to obtain authorizations for identified services.
- Identifies and utilizes both formal and informal community supports and networks.
- Attends community-based meetings with the Program Manager for collaboration with other organizations and services.
- Performs day-to-day office operation duties as assigned.
- Attends agency in-services, workshops and seminars as required.
- Participates in the agency’s utilization review, incident reporting and quality improvement program as deemed appropriate by the supervisor.
- May need to carry a small caseload for service delivery to families.
Education/Experience:
- A Bachelor’s Degree in human services, business or comparable field.
- At least two years related experience preferred; or equivalent combination of education and experience.
- A valid NYS Driver's License is required.
