The RN in Home Health provides skilled nursing care to patients in their home setting, typically after hospitalization, surgery, or for chronic health conditions. The RN is responsible for assessing patient needs, creating care plans, providing treatments, and educating patients and families on health management.
Key Responsibilities:
- Patient Assessments:
- Conduct initial and ongoing assessments of patients’ physical, emotional, and psychological needs.
- Monitor vital signs, medical history, and other indicators to evaluate the patient's health status.
- Assess patients for signs of complications, infection, or worsening conditions.
- Care Plan Development:
- Collaborate with the patient’s physician and other healthcare providers to create an individualized care plan.
- Adjust care plans based on patient progress or changes in condition.
- Educate patients and their families about care plans, treatment options, and expected outcomes.
- Medical Treatments & Interventions:
- Administer medications, injections, and IV therapy as prescribed.
- Provide wound care, catheter care, and other nursing procedures.
- Monitor and manage chronic conditions such as diabetes, hypertension, or respiratory issues.
- Perform diagnostic tests, lab work, and health screenings as needed.
- Patient Education:
- Educate patients and families on managing conditions, medications, nutrition, exercise, and lifestyle changes.
- Teach proper techniques for wound care, medication administration, and use of medical equipment.
- Ensure the patient and family understand the signs of complications and know when to seek help.
- Care Coordination:
- Act as a liaison between the patient, family, physicians, and other healthcare team members.
- Coordinate home health services, such as physical therapy, occupational therapy, or medical social services, when necessary.
- Ensure continuity of care by documenting patient status, treatment provided, and any changes to care plans.
- Documentation:
- Maintain accurate and detailed records of patient visits, treatments, and progress in compliance with healthcare regulations.
- Ensure all necessary documentation for insurance billing, reimbursement, and regulatory compliance is completed.
- Emergency Response:
- Be prepared to recognize and manage medical emergencies or deteriorating conditions, coordinating with emergency services when needed.
- Monitor patients' health progress and communicate effectively with the healthcare team regarding changes in patient condition.
- Follow-Up Care:
- Schedule and conduct follow-up visits to evaluate patient progress and reassess care plans.
- Ensure all necessary supplies and medications are provided for continued home care.
Qualifications:
- Education:
- Associate’s or Bachelor's degree in Nursing (ADN or BSN).
- Licensing:
- Current RN licensure in the state of employment.
- Certification:
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications may be required.
- Experience:
- At least 1-2 years of nursing experience, preferably in a clinical, home health, or hospice setting.
Skills:
