A Rehabilitation Registered Nurse (RN) provides nursing care to patients recovering from surgery, injury, or illness in a rehabilitation setting. These patients may have physical, emotional, or cognitive impairments and require specialized care to regain independence and improve their functional abilities. The role involves working with a multidisciplinary team, including physical therapists, occupational therapists, and speech therapists, to create and implement individualized care plans for each patient.
Key Responsibilities:
- Patient Assessment & Care Planning:
- Conduct comprehensive nursing assessments of patients’ physical, emotional, and cognitive status upon admission and throughout their rehabilitation process.
- Develop, implement, and update individualized care plans in collaboration with the healthcare team, focusing on the patient's rehabilitation goals and needs.
- Regularly evaluate patient progress, documenting improvements or setbacks, and adjusting care plans accordingly.
- Monitor patients for changes in condition and report findings to physicians and the rehabilitation team.
- Medication Administration & Management:
- Administer prescribed medications, including pain management and medications related to rehabilitation, as well as any other treatments ordered.
- Monitor patients for side effects or complications related to medications, adjusting administration as necessary.
- Ensure proper medication management, including accurate documentation and maintaining patient safety protocols.
- Monitoring and Health Management:
- Regularly monitor patients’ vital signs (e.g., blood pressure, heart rate, temperature) and assess for any changes that could affect the patient's rehabilitation progress.
- Observe and assess for signs of complications such as pressure ulcers, deep vein thrombosis (DVT), or infection, and intervene as necessary.
- Provide wound care, including dressing changes and monitoring healing, for patients recovering from surgery or injury.
- Collaborative Care:
- Work closely with physical therapists, occupational therapists, speech therapists, dietitians, and other healthcare professionals to coordinate and provide holistic care for the patient.
- Participate in daily interdisciplinary team rounds to discuss patient progress, address concerns, and adjust rehabilitation goals.
- Communicate clearly with patients and their families about rehabilitation progress, goals, and challenges, offering emotional support as needed.
- Patient Education & Support:
- Educate patients and their families on rehabilitation procedures, exercises, and safety protocols.
- Provide guidance on post-discharge care, including strategies for continued recovery at home, medication management, and follow-up appointments.
- Teach patients how to use adaptive equipment, assistive devices, or mobility aids as part of their rehabilitation process.
- Promote Mobility and Function:
- Assist with exercises, mobility training, and range-of-motion activities, helping patients regain strength, coordination, and independence.
- Monitor and guide patients through activities of daily living (ADLs), such as bathing, dressing, eating, and personal hygiene, as part of the rehabilitation process.
- Help patients set and achieve rehabilitation goals, fostering independence while providing support when needed.
- Safety and Infection Control:
- Ensure a safe rehabilitation environment by following infection control guidelines and maintaining a clean and organized area.
- Implement and monitor fall prevention strategies, positioning techniques, and the proper use of mobility aids to ensure patient safety.
- Monitor for any signs of complications related to immobility or prolonged recovery, such as pneumonia, pressure ulcers, or aspiration.
- Emotional and Psychological Support:
- Provide emotional support to patients and their families, addressing the challenges of the rehabilitation process.
- Recognize signs of depression, anxiety, or other mental health issues related to recovery, and refer patients to appropriate resources or counseling services.
- Documentation and Reporting:
- Accurately document patient assessments, care plans, treatments, medications, and progress in patient records.
- Ensure all documentation is in compliance with legal, ethical, and facility-specific standards and regulations.
- Report any significant changes in the patient’s condition or concerns to the attending physician and the rest of the care team.
Skills & Qualifications:
- Education:
- Bachelor’s or Associate’s Degree in Nursing (BSN or ADN) from an accredited nursing program.
- Licensing:
- Active Registered Nurse (RN) license in the state of employment.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications may be required depending on the facility.
- Experience:
- Experience in a rehabilitation or acute care setting is highly preferred, particularly with patients recovering from surgeries, strokes, traumatic injuries, or neurological conditions.
- Knowledge of rehabilitation therapies, adaptive equipment, and mobility aids is beneficial.
- Skills:
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Ability to collaborate effectively within an interdisciplinary healthcare team.
- Excellent communication skills to interact with patients, families, and other healthcare professionals.
- Empathy and patience in working with patients facing physical and emotional challenges.
