What are the responsibilities and job description for the Care Coordinator position at ICL Inc.?
Institute for Community Living (ICL) is a non-profit organization servicing the NYC-Metro area and for over 35 years, we’ve provided life-saving care to New Yorkers in need that includes but is not limited to addiction assistance, mental health services, and housing support for all. So, it is our pleasure to announce that we are currently seeking a full-time candidate for the role of Care Coordinator to join our Pathways to Wellness Health Home team!
By ICL definition, a Care Coordinator (CC) is a trained health professional who is responsible for assisting and managing a patient's care. Functioning, as a member of an interdisciplinary team, C.C.s are expected to provide clinical support by providing consultation, education, interventions, and resources to maintain focus on outcomes and best practices, with the focus being to provide linkage to services to decrease high-risk medical and/or psychiatric hospitalizations.
To be considered for this role, you MUST first adhere to and understand the requirements and tasks listed below…
MINIMUM REQUIREMENTS:
- Available Monday to Friday – Day Shift: 8:30am - 5pm
- Location: Staten Island, NY 10312 (Main Base) – S.I. residents are preferred but not mandatory
- VALID Driver’s License Photo ID – NYS preferred
- TRAVEL: up to 90% of the weekly work schedule will involve fieldwork – in rear instances, an agency vehicle may be provided, otherwise, various modes of reliable and safe transportation use is a MUST
- Knowledge of Medicaid, Social Security, and other entitlements – preferred
- Knowledge of the interdisciplinary planning process and the developmental treatment model
- Knowledge of computer software and electronic health record systems – e.g. EPIC, FCM, HCS, etc. are_ preferred but not mandatory_
- Following all safety guidelines in accordance with the performance of tasks mentioned
- Strong communication, interpersonal, writing, and conveying skills – bi-lingual is a plus but not mandatory
- Proper attire at all times – this includes business casual and/or professional per the ICL dress code policy
MINIMUM EDUCATION & EXPERIENCE REQUIREMENTS:
- Bachelor’s Degree or higher in any of the following… social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing, or other human services field
- PLEASE NOTE: In substitute for a degree, ICL will accept the following, in addition to… NYS teacher’s certificate; or NYS licensure & registration as a Registered Nurse (RN); or Credentialed Alcoholism and Substance Abuse Counselor (CASAC) ...PLUS...
- 2-5 years of direct clientele service – e.g. serious mental illness, developmental disabilities, or substance abuse disorders preferred; or
- 2-5 years of linking clientele to a broad range of services essential to successful living in a community setting – e.g. medical, psychiatric, social, educational, legal, housing, financial services, etc.
CARE COORDINATOR TASKS:
- Provides crisis intervention and follow-up
- Provide care to a variety of clients with few and/or multiple medical conditions that include but are not limited to medical comorbidities and co-occurring substance abuse disorders and so on
- Advocates and supports the client's engagement with community agencies, health care providers, and others on their behalf to ensure access to services needed to help increase wellness self-management and reduce emergency room visits and/or hospitalizations
- Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals, and resources
- Participates in the development, documentation, review, and update of client-centered comprehensive integrated, interdisciplinary care plan in consultation with other team members to ensure focus on desired outcomes
- Maintains effective communications with clients, primary care physicians, substance abuse, and mental healthcare providers, family, collateral resources, and other agency staff on behalf of clients
- Maintains effective documents, records, statistics, and other related reports in an organized, timely, and accurate manner as per policy and procedure
- Coordinates care planning with other providers of services/ resources to ensure goal-directed, collaborative care, including care transitions
- Attends and participates in team meetings to provide input and feedback around the following to review client status, update plans and goals, and review outcomes to further program goals - psychosocial, substance abuse issues and resources, medical conditions, comorbidities, and so on
- Provides telephonic as well as face-to-face outreach, engagement, and service planning in the field
- Acts as a linkage to community services including medical, behavioral, residential, entitlement, and any other needed services per interdisciplinary care plan.
- Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers and resources as needed.
- Perform other job-related tasks as assigned in an organized and detail-oriented manner
TRAINING REQUIREMENTS
- Specific training for the designated assessment tool(s), the array of services and supports available, and the client-centered service planning process
- Mandated training on the NY State Community Mental Health Assessment instrument and additional required training
- Training in the assessment of individuals whose condition may trigger a need for HCBS support and ongoing knowledge of current best practices to improve health and quality of life
