Description
General Summary: A nonexempt position responsible for performing a variety of duties. Involved in both the clinical and administrative areas including assisting physicians with patient care and handling environmental and organizational tasks. Provide information to patients so they may fully utilize and benefit from the clinical services. The LPN functions within the scope of practice as defined by the New Jersey Board of Nursing.
Reports to: Nursing Manager/Sr. Director of Clinical Services
Job Supervisory Responsibilities: The LPN may assume some duties of the office clinical staff with the oversight of an RN or physician.
Essential Job Responsibilities:
- Obtains, verifies, and records health history of patients. Gathers pertinent health data through physical assessments, patient records, and collects specimens for laboratory analysis. Documents patient visit information including medication, allergy, and patient history in EMR. Refills medications per physician.
- Provides basic patient care as ordered. Administers medications and treatments as directed. Assists other health care professionals in performing exams, diagnostic procedures and treatments, and minor medical procedures. Adheres to ID Care infection control/safety guidelines and confidentiality policies. Explains treatment and procedures to patients; relaying instructions to patients/families; answering and directing patient calls and providing pertinent information.
- Fulfills environmental responsibilities as assigned that may include setting up equipment according to ID Care and department protocols; cleaning exam rooms and equipment between patient visits to maintain infection control; ordering, sorting, storing supplies; and restocking exam rooms.
- Documents assessments, interventions, health histories, patient/family responses, medications dispensed/prescribed, and test results in medical records.
- Promotes wellness by providing patient education materials, communicating physician’s advice/instructions.
- Fulfills clinical responsibilities which may include physical examination preparations; drug administration through various routes including injections; prescription verification with physician’s orders, applying dressings, biohazard waste disposal and monitoring; laboratory procedures and quality control methods according to Occupational Safety and Health Administration (OSHA) guidelines; CLIA-waived testing; venipunctures; specimen handling such as urine, throat, vaginal, stool, and sputum.
- Reinforcing patient and family teaching and provision of supportive care under the direction of a registered nurse or physician.
- Other duties as assigned.
Equipment Operated: Standard nursing equipment and supplies including B/P machine, thermometer, oxygen tank, AED, syringes, pulse oximetry unit, Abbott ID NOW COVID instrument, and computer.
Work Environment: Nurse's office and medical exam/treatment rooms. Controlled lighting, air quality and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.
Requirements
Education and Experience:
- High School Diploma or equivalent. Graduate of an approved practical nursing program.
- Minimum of 1 year of LPN experience; clinic experience preferred.
Other Requirements:
- State-licensed practical nurse
- Current CPR certification
- Traveling to local offices.
Mental and Physical Requirements: Involves standing, walking, bending, grasping, manipulating and squatting. Occasional need to lift patient during transfer/turn process with or without assistance.
Other Requirements:
- Influenza: All employees of ID Care shall be provided the influence vaccine. Employees will be required to obtain the influenza vaccine by December 1 of each calendar year.
- Covid: As of 8/1/2021, current ID Care employees will be required to be fully vaccinated. Employees may receive the vaccine from a provider, pharmacy, or outside health facility of choice. Staff are considered fully vaccinated: • 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or • 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine.
- Effective 1/3/2022 Administration of an additional dose of vaccine 2 months after a JNJ vaccine and 6 months after an mRNA vaccine (Moderna or Pfizer) are required to be fully vaccinated.
