Remote Clinical Social Worker at Intermountain Health in Boise, Idaho, United States Job Description Job Description : The Connect Care- Behavioral Health Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, implement plans of care with the appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide individual treatment as indicated. This position works collaboratively with patients, their family and / or caregivers, and providers so they can self-manage their care, optimize their functional health status, have quality outcomes, and use the health system appropriately. Develop, document, and implement plans which will provide the appropriate resources to address social, physical, mental, emotional, spiritual, and supportive needs. Facilitate virtual group therapy. This role will be a part of the Connect Care- Behavioral Health Team. Looking for a candidate with experience in the treatment of adolescents and / or pain management. Will accept LCSW or LMFT licensure. Salary will be based on licensure and experience. Minimum Qualifications Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified. Current State Licensure as LCSW or LMFT. Computer Skills, i.e., email, word processing. Excellent written and verbal communication skills. Preferred Qualifications : Experience with patients, specifically adolescents and / or pain management, and treatment teams . Physical Requirements : Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
