We’re seeking a Urology Clinical Navigator to join our team at Idaho Urologic Institute. In this role, you’ll provide personalized care coordination and education for patients with urological conditions, empowering them to make informed decisions and achieve the best outcomes.
Key Responsibilities
- Patient Navigation: Serve as a consistent point of contact for patients from diagnosis through treatment and follow-up, specializing in conditions such as prostate cancer, kidney stones, incontinence, and more.
- Education: Provide tailored education on urological conditions, treatment plans, surgical options, and post-care management.
- Care Coordination: Schedule and track appointments, procedures, imaging, and lab work to ensure timely and seamless care.
- Emotional Support: Offer empathetic support to patients and families, addressing concerns and connecting them with resources.
- Clinical Collaboration: Work closely with urologists, oncologists, radiologists, and other healthcare professionals to optimize care plans.
Qualifications
- Active LPN license or CMA/RMA.
- Minimum 2years of clinical experience, preferably in urology, oncology, or case management.
- Strong knowledge of urological conditions, treatments, and patient education strategies.
- Exceptional organizational skills and ability to manage multiple patients and priorities.
- Proficiency in EMR systems.
- Experience in pre-authorization, particularly for high dollar, complex medications.
