Location : Everett, WA
Compensation : $22.00-$24.00 per hour
Schedule : Monday-Friday, 40 hours per week

The Role :
The Spoke Care Navigator , plays a key role in working closely and collaboratively with other Care Navigators to coordinate patient care, keep the patient engaged with services, address issues related to relapse, and communicate together on patient needs. This role works under general direction of the Hub Administrator and Hub Care Manager in providing services to the Multicare Health System Hub & Spoke program, and as a part of the Ideal Option team. Additional responsibilities include :

Has a knowledge of substance use disorders and their effects on individuals, families, and communities. Is able to communicate effectively with patients and within the organization and community agency members. Is able to adapt to individual needs. Good at problem solving, decision making and upholds the principles of ethical conduct.
