Home Based Case Manager

Job Overview:
We are seeking a dedicated and compassionate Home - Based Case Managers to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients' rights and well-being. Duties:

Services are outcome focused, needs based activities which assist families and children by locating, coordinating, and monitoring care and services. Services may be provided by Master or Bachelor level clinicians.

Qualifications:
- Bachelor's degree in social work or a related field

-Associate Degree, High School Diploma, or GED, they must also have: At least 4 years full time employment experience providing direct casework services to children & families that includes providing services to families that need assistance in the protection and care of their children

- Previous experience in case management or a related field is highly desirable
- Knowledge of child welfare policies and procedures
- Familiarity with motivational interviewing techniques
- Strong organizational skills with the ability to prioritize tasks effectively
- Excellent communication skills, both written and verbal
- Ability to work collaboratively with a diverse range of individuals and organizations
- Proficiency in Microsoft Office Suite
