If you are seeking an opportunity to join a large organization and work within a team-oriented culture, please apply today!Position Overview / Responsibilities for the Case Management Assistant : Collaborate with Case Managers and Social Workers to assess patient needs and develop discharge plans Ensure all necessary referrals, authorizations, and transportation arrangements are completed Maintain accurate and timely documentation in the electronic medical record (Epic) Manage clerical tasks such as copying, faxing, scanning, and data entry Participate in daily team huddles and department meetings to share knowledge and resolve issues Required Skills for Case Management Assistant : Proficiency with Epic EHR Strong oral and written communication skills Ability to work independently with minimal supervision Intermediate PC skills, including word processing and data entry Exceptional time management and problem-solving skills What's in it for you?
