Position Details : Monogram Health Remote RN RoleKey Highlights : 100% VirtualConducts Annual Wellness Exams and focused screenings.Assists patients in improving health outcomes through high-quality virtual care.Key Responsibilities : Educate patients and families on available health programs and benefits.Document important patient information, including : Chief complaintsDiagnosesMedical, family, and social historyMedications, allergies, assessments, and care plansEnsure accurate and timely documentation.Maintain high-quality care standards.Assist patients with enrolling in educational health programs.Conduct health assessments and follow up on patient progress.Participate in integrated care team meetings.Orientation Details : Duration : 4 days in-person at the Brentwood office (Nashville, TN).Schedule : Monday – Thursday at 8 : 30am-5pm (Thursday 3pm departure).Travel Requirements : Clinicians must fly in on Sunday afternoon or evening before training.Clinicians will fly out on Thursday evening or Friday, depending on flight availability.Additional Virtual Training follows after in-person sessions.Travel Arrangements : Clinicians arrive Sunday afternoon.Depart Thursday evening or Friday morning, depending on flights.Flights reimbursed by Monogram Health.Hotel accommodations are provided by Monogram Health.Meal reimbursement up to $50 / day while in Nashville.Technology Provided : Laptop issued at orientation in Nashville.No additional technology provided.Start Date Options : Training sessions available every two weeks.Next start date : 3 / 3 / 2025.Candidates must align with one of these dates.Candidate Requirements : Must reside in one of the following states : Florida, Georgia, Iowa, New Jersey, Ohio, Pennsylvania, Texas, Tennessee, Mississippi, Louisiana, Alabama. Preferred Experience : Case Manager, Telehealth, Home Health, Palliative Care, Hospice, Dialysis, or Nephrology.Experience caring for patients with chronic diseases, CKD / ESRD.Minimum of 2 years RN experience.Technology Proficiency : Strong internet and phone connection for remote work.Comfort with Athena & Salesforce for documenting patient visits.Familiarity with MS Office, mobile devices, and various technology tools.Work Schedule : Monday - Friday, 8 AM - 5 PM (local timezone).Ability to implement a care plan aligned with patient needs.
