Job Summary
The Social Worker is responsible for facilitating a positive teaching and training atmosphere conducive to resident health and well-being by leading the interdisciplinary team in the development and implementation of person-centered plans and related programming and documentation efforts. The Social Worker is responsible to meet all Qualified Intellectual Disabilities Professional (QIDP) requirements for assigned programs.
Duties and Responsibilities
E- Connotes Essential Functions
E-1 Provides QIDP functions for assigned programs.
E-2 Assesses all sections of the Comprehensive Functional Assessment for assigned participants annually and as needed.
E-3 Reviews all clinical interventions as necessary to meet the needs of assigned participants.
E-4 Oversees the development of in-service training for staff in regards to all residential, clinical, and medical interventions.
E-5 Participates as the leader of the Interdisciplinary Treatment Team – oversees all residential, medical, and clinical documentation for the program.
E-6 Participates in individual admission, movement, and discharge procedures as needed.
E-7 Participates in projects and committees as needed such as team meetings, special ITT’s and clinic meetings.
E-8 Provides documentation training and consultation for residential personnel in regards to suggestions, ideas and information for developing and writing programs or other interventions.

E-9 Ensures required information is maintained in residential records.
E-10 Maintains care plans/Plans of Nursing Services.
11- Interacts with participants, clinical staff and other agency staff in a positive manner to facilitate the development of good working relationships and enhance participant relationships and skill development.
12- Maintains survey readiness though ongoing monitoring and tracking procedures and the completion of quarterly and as needed documentation reviews
13- Maintains safety standards and follows all IGHL safety policies.
14- All other duties as assigned.
Education, Work Experience, and Competencies
· Master’s degree in Social Work from an accredited program
· One-year experience working or training in the field of Human Services
· Good oral and written communication skills
· Good interpersonal skills
· Valid NYS driver’s license
· Experience working with people with ID/DD
Knowledge of QIDP requirements/regulations
Job Type: Full-time
Pay: $29.00 per hour
Expected hours: 40 per week
