Position Overview

The Division of Mental Health is seeking to hire a Social Worker III for the Elgin Mental Health Center located in Elgin, Illinois to function as the designated leader for a professional team, guides, advises and coordinates the professional interdisciplinary team in the formulation and administration of recipient care and treatment/habilitation. Conducts involved casework interviews, works with relatives, agencies, and other appropriate resources to obtain pertinent information for the purpose of identifying psychosocial stressors and treatment problems.

Essential Functions

  • Conducts involved casework interviews, works with relatives, agencies, and other appropriate resources to obtain pertinent information for the purpose of identifying psychosocial stressors and treatment problems.
  • Functions as the designated leader for a professional team, guides, advises and coordinates the professional interdisciplinary team in the formulation and administration of recipient care and treatment/habilitation.
  • Relates to community agencies, funding sources and other appropriate resources for the purpose of effective discharge planning and implementation.
  • As monitoring professional, reviews treatment plans of inter-disciplinary treatment team members.
  • Functions as the social work resource person, including having a familiarity with community resources and treatment modalities for the treatment team.
  • Participates in the program-wide and hospital-wide professional social work group.
  • Participates in Quality Assurance for the unit and discipline. Tracks data regarding treatment plans, problem intervention documentation and patient participation in treatment planning.
  • Performs other duties as required or assigned which are reasonably within the scope of those duties enumerated above.

Minimum Qualifications

  • Requires a master's degree in social work from a recognized college or university supplemented by two (2) years of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled, or other disabled persons.

Conditions of Employment

  • Requires the ability to walk and stand for a significant period of time, including stooping, bending, lifting or exerting up to 20 pounds of force occasionally.
  • Requires the ability to physically restrain patients as necessary to prevent injury to patient or others.
  • Requires the ability to utilize office equipment, including personal computers.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
