Job Overview

We are seeking a skilled Palliative Nurse Practitioner to join our team at Illumia. As a key member of our healthcare staff, you will play a vital role in providing comprehensive care to patients with advanced illnesses.

About the Role

The successful candidate will have a strong background in nursing and experience working in palliative care settings. You will be responsible for developing individualized care plans, collaborating with healthcare teams, and ensuring effective pain and symptom management for patients.

You will work closely with patients, families, and caregivers to understand their unique needs and preferences, and provide compassionate and patient-centered care that addresses their physical, emotional, and spiritual well-being.

Responsibilities

  • Develop and implement individualized care plans for patients with advanced illnesses.
  • Collaborate with healthcare teams to ensure seamless coordination of care and effective pain and symptom management.
  • Provide education and support to patients and families on managing chronic conditions and end-of-life care.
  • Participate in quality improvement initiatives to enhance the delivery of palliative care services.

Requirements

  • Masters of Science in Nursing from an accredited school of nursing.
  • Minimum two years of experience as a Nurse Practitioner in palliative care or a related field.
  • Active and unrestricted Nurse Practitioner certification/license without board actions.
  • Ability to work autonomously in an Advanced Practice Role.
